I/We the parents of the above camper(s) understand that if our child (children) is absent for any reason (sickness, vacation, other commitments, etc) we are responsible for paying for all weeks we commit to at the time of registration. I/We understand fees will be collected every Monday with the exception of Monday, May 31st due to the Memorial Holiday. Weeks of May 31st & July 4th will not be pro-rated.

I/we, the participant and parent (s), do hereby release MVH LLC dba Arctic Edge Ice Arena and its directors, counselors, staff, and instructors of personal property losses, accidents, or injuries of any kind occurring while I/my child is participating in any and all programs, acknowledging that each will inspect the facilities and equipment to be used, and if they believe anything is unsafe, they will immediately advise their coach, counselor, or supervisor of such conditions and refuse to participate.

A $25 non-refundable registration fee is required for each camper. The registration fee will hold your child's spot in our camp. Weekly fees including lunches will be collected every Monday of camp, for that week.
